Professor Keri Thomas presents on identifying, assessing and planning for patients reaching the end of life.
Learning objectives

This module will advise on primary care management of end-of-life patients, including:

  • How the Gold Standards Framework aids end-of-life care
  • Identifying 'the 1%'
  • Reducing hospitalisations

Professor Thomas gave this talk at Pulse Live which took place in May 2013.
